Haryana Forms PDF Download: Pension, Compensation and Government Forms

Common citizens, farmers and CSC / Atal Seva Kendra operators in Haryana are often troubled by the same question — where can the correct government form be found? This page brings together in one place the official portals for Haryana’s most important forms. Please note: for some tasks there is no permanent blank PDF form at all; that work is done directly on the online portal. In such cases, it is clearly written below why there is no PDF and which portal is the correct one. Never share your OTP or bank details with any unknown person for any form.

1. Parivar Pehchan Patra (PPP) Correction / Grievance — Online Only

Who this form is for: Families whose Parivar Pehchan Patra (PPP / Family ID) has incorrect name, date of birth, address, income or member-related information, or who need to file a complaint related to PPP.

Who can apply: Any adult member of the family, with their Family ID and registered mobile number.

Documents required: Family ID, Aadhaar card, and proof of the correct information (such as birth certificate, address proof, income proof — whichever relates to the correction).

Where and how to submit: There is no single permanent blank offline correction PDF for PPP correction. Correction is done online only — through the ‘Update Family Details’ option on the official PPP portal, and complaints through the PPP office’s grievance module. You can also visit the nearest Atal Seva Kendra (CSC) for help.

2. Haryana Domicile Certificate and Caste Certificate

Who this form is for: A domicile certificate proves that the applicant is a resident of Haryana; a caste certificate is issued for Scheduled Caste / Backward Class and other categories. Both certificates are required for jobs, admissions, scholarships and many government schemes.

Who can apply: Any eligible citizen living in Haryana; for a caste certificate, belonging to the relevant category is necessary.

Documents required: Family ID (Parivar Pehchan Patra), Aadhaar card, address proof, birth / age proof, and the relevant proof for caste. A pre-verification report is also required — in rural areas from the sarpanch / numberdar and patwari, and in urban areas from the ward member and municipal officer.

Where and how to submit: The application is made online through the Antyodaya SARAL portal; a blank form can be obtained from SARAL Seva / tehsil / CSC. No fee amount is written on this page, because it should be seen on SARAL’s live service page itself.

3. Old Age / Disability / Widow Pension Application

Who this form is for: Old Age Samman Allowance (senior citizens), widow and destitute women pension, and disability pension — these are all schemes of the Haryana Social Justice and Empowerment Department.

Who can apply: Senior citizens, widows / destitute women and persons with disabilities who meet the eligibility conditions of the scheme.

Documents required: Aadhaar card, age proof, ration card, bank account details, and where applicable income proof and disability certificate.

Where and how to submit: Forms can be taken from the Social Justice Department’s official forms page, and the application can be made through CSC / Block Development and Panchayat Officer (BDPO) / municipal office. No pension amount is written on this page — take the amount only from the department’s live official information.

4. Kisan Credit Card (KCC) Application

Who this form is for: For a Kisan Credit Card to take short-term credit for farming (fertiliser, seed, irrigation and other farming expenses).

Who can apply: Individual farmers, joint farmers and farmer groups; for PM-KISAN beneficiaries, a simplified form facility is stated to be available at the bank.

Documents required: Aadhaar card, land records / khasra, passport-size photo, and other documents asked for by the bank.

Where and how to submit: KCC forms are bank-wise — there is no single ‘Haryana KCC form’ PDF for all banks. So take the form from your own bank branch, fill it there and submit it there. Sharing any one bank’s PDF as ‘Haryana’s KCC form’ would be wrong.

5. Crop Loss (Kharaba) / Crop Loss Claim — e-Kshatipurti, Online

Who this form is for: For claiming compensation when a crop is damaged by flood, hailstorm, drought or other natural calamity.

Who can apply: Farmers whose crop is registered on Meri Fasal Mera Byora and whose crop has been damaged.

Documents / information required: Meri Fasal Mera Byora registration details, field / crop details, and information related to the loss; verification is done through the patwari and payment is made by DBT.

Where and how to submit: There is no permanent offline kharaba PDF for this. The claim is made online on Haryana’s e-Kshatipurti portal, and before that the crop must be registered on Meri Fasal Mera Byora. Please note: a crop insurance (PMFBY) claim is a separate process.

6. District Court (Peon / Process Server) Offline Vacancy Form — There Is No Permanent Form

Who this form is for: For recruitment to Peon, Process Server and other posts in district courts.

Who can apply: Only candidates who meet the eligibility, age and educational conditions of that district’s current notification.

Documents required: Documents asked for in the notification — usually educational certificates, age proof, caste certificate (if applicable) and photo — but always take the final list from the current notification itself.

Where and how to submit: Every district (Rohtak, Hisar, Panipat, Yamunanagar etc.) issues its own separate notification and its own separate application proforma; old notifications expire. So do not fill an old notification from 2025 or early 2026 treating it as the current form. Always see the current notification in the Recruitment / Notice section of the concerned district’s official district court website and fill the proforma given with it. For example, the Recruitment section of Rohtak District Court is given below — at the time of checking, no current notice was listed there, which shows that a form need not be available at all times.
